Advance Directive / Health Care Directive

An advance directive lets you name someone to make health care decisions if you cannot speak for yourself.

In New Hampshire, this usually also includes living will instructions about serious illness and end-of-life care. It is different from a financial power of attorney, and it does not control who receives your property after death.
